Acceptable Documents for Birth Verification

The following documents can be used for birth vertification when registering with Markham Minor Lacrosse

1. Birth Certificate
2. Health Card (green style showing age, not red and white)
3. Passport, current or expired
4. Baptismal Certificate
5. Confirmation Certificate
6. Doctor's Note showing physicians letterhead
7. School Note from Principal showing school letterhead

Risk and Release of Liability Policy

It is a requirment for 2008 by the Ontario Lacosse Association that the a waiver be signed called the Acknowledgement of Risk and Release of Liability for all players that are participating in the sport of Lacrosse. This form MUST be signed during the registration process in order to complete the registration. So please take the time NOW to review and understand the waiver before you registration. If the player or Guardian does not sign the waiver the player is NOT considered registered.

Refund and Cancellation Policy

As association we implemented policies and incentives to encourage our players to register early. The refund policy along with early bird discount and late registration fees are all part of these policies. Late registrations and withdrawals cause a massive amount of extra work for association administrators as well as additional costs. In some cases we are left with extra floor time that we can't fully sell or return to the city. Adding or subtracting teams or players on teams causes problems with balancing and jerseys. With all of this in mind, our refund policy is as follows.

If you cancel:

Prior to March 15: Full Refund
• Between March 15 and the start of the season, April 15: Registration Fee minus $25.00 Admin Fee = Amount of Refund
• After the start of the season, April 15: No Refund.

ALL requests for refunds must be made in writing to the Registrar.

Bounced Cheque Policy

All payments that do not clear correctly through the appropriate bank procedures are subject to a $25.00 administration fee.

Release Policy

Releases will only be considered for players who apply in writing and who state:

• the reason for the request
• full mailing address including postal code

It is a requirment for 2008 by the Ontario Lacosse Association that the form Interim Release Request Form must be submitted with every release request.

This release must be acquired from the Markham Minor Lacrosse Association in order for the player to register elsewhere. A release will be granted for one year only (conditional) with the player returning to Markham the following year or requesting a second (unconditional) release to continue playing elsewhere (even if it is to a different center).

Should a release be granted, it will apply only to the organization named on the application.

No releases will be granted from the Markham Minor Lacrosse Association until:

• all outstanding monies are paid in full and
• all equipment belonging to the Markham Minor Lacrosse Association has been returned.

The Markham Minor Lacrosse Association Executive reserves the right to review each application on an individual basis.
